To be visible in the Interview screen, questionnaires must be linked to a specific project.  This allows the same questionnaire to be used from one project to the next.  Linking is done via the Projects menu option, which is available from the Time & Exp left menu option (given sufficient access exists).

Edit the project record and use the multi-select to link one or more questionnaires to the project.
