Request Questionnaires

Questionnaires are designed in order to allow the collecting of detailed information for requests.  A questionnaire is a group of questions that together detail the minimum information that must be gathered in order to complete a specific request.  Each catalog item can be assigned to a given questionnaire.  Questionnaires, in turn, are assigned one or more questions.  This allows the same questionnaire to be used for multiple catalog items.
